COVID-19 Vaccine
You may receive a letter from NHS England inviting you for your covid vaccination. You will have the option to have a jab at community vaccination centers and even some pharmacies. Our commitment at SFD is to ensure all of our over 70s and shielding patients are vaccinated before the 15th February. We are quite happy for patients to make their own arrangements with these community centers via the National system, but for those who don't, we will be in touch with you to offer you an appointment within then next three weeks.
In the meantime please could we kindly ask you not to contact the practice we will contact you to make an appointment and it may stop someone who needs urgent medical help getting through to us.

Covid-19 Update
Due to the current epidemic we are not accepting online bookings for appointments. Please complete an E-Consultation form and submit online. We will respond to you by no later than the end of the next working day. Alternatively please contact the Surgery by telephone
New Opening Hours - Extended Access
From April 2018 we will be extending our service. In addition to the early morning hours that we currently provide on Tuesday and Wednesday mornings we will also be providing evening appointments from 6.30pm - 8.30pm on Wednesday evenings at our Denby Dale Surgery. These will be a mixture of telephone, on the day and pre-bookable appointments. Clink the link above to check availability and book an appointment today. We hope that this will help to improve access to our service, especially for those who have difficulty attending appointments during the working day.
Please note that all incoming and outgoing calls to and from the practice are recorded for training and monitoring purposes

Named GP
All patients at Skelmanthorpe Family Doctorshave a named, accountable GP who is responsible for coordinating their care. Your named GP will be allocated to you by the practice. You can still make appointments to see any GP or Nurse of your choice, not just your named GP. If you wish to know who your allocated GP is, then please call the practice and a member of our Reception Team will be able to advise you. If you have a preference and would like to request a particular doctor at the practice to be your named GP please inform one of our Reception Team.
Carers Champions
We keep a register of patients, who, without payment, provide help and support to a family member, friend or neighbour who cannot manage on their own due to physical or mental illness, disability, substance misuse or frailty brought on by old age. People who care for someone in these circumstances, without pay, are considered to be 'carers'.
We have two Carer's Champions at the Practice, Liz and Sally and who can help provide you with relevant information, resources and contact points. We have a dedicated notice board where up to date information is displayed in the practice. Please visit our Carers page for more information.
Access
Skelmanthorpe Health Centre and Denby Dale Surgery have disabled access and toilet facilities. Skelmanthorpe Health Centre has a car park.
Mentoring & Research
The Practice participates in the mentoring of Health Care Professionals and is engaged from time to time in clinical research. Patient consent is sought prior to any research activities been undertaken.
