New Patient Registration
As part of the registration process, it is Practice policy the registration procedure, for all new patients aged five and over to see the practice nurse for a new patient health check. This includes checking your blood pressure, urine, measuring your height and weight, giving advice on a healthy lifestyle, recording any current problems and/or allergies in your medical history.
For each person wishing to register with us we require either a medical card with the section “what to do if you change your doctor” completed, or alternatively a completed GMS 1 form, available from reception.
We also require two forms of identification; one photographic (ideally a passport or driving licence) together with a copy of a utility bill/bank statement.
Change of Personal Details
It is very important that the Practice has up-to-date records. We would be very grateful if you could advise reception if you change either your address and or telephone number.
