Supporting Self Care Week 2018

Supporting Self Care week (12-18th November 2018) 

There are a range of resources available via the Self-care Forum website, from simple TV screen messages, posters, banners and website items.  Self-care is one of the few genuine routes to managing demand and is elemental in the signposting work.  Self Care week provides a nudge to promote self-care further.

Last year social media and leaflets, such as minor ailment guidance, was most successful in delivering the self care messages. For the practice it is an opportunity to reach a consensus on which conditions you can consistently advise self care, involving your local pharmacist where appropriate.  For patients with Long Term Conditions you can promote online access to their full medical record to gain more engagement with self-care and self-management as well as medication compliance. A patient-derived care plan also supports self-management.

Options include promoting winter health, the skills of the community pharmacist, promoting NHS choices for advice on many ailments but also include prioritising good quality decision making for patients through information and brief decision aids. and are good websites which provide brief decision aids for many common conditions.
